This is an hourly consumer-directed employment model, not a daily stipend to a live-in caregiver and not an agency that hires your relative for you. A financial management or fiscal agent handles payroll, tax withholding, employer filings and issuing the caregiver’s paycheck, so the family is not writing checks or filing employment taxes alone.

Who Can Be Paid Under Hawaii Family Caregiver Pay
Under QUEST Integration Self-Direction the member is the employer and may hire family members, neighbors and friends as their paid workers. Med-QUEST’s waiver documents state that beneficiaries may hire relatives including spouses, adult children and parents of adult beneficiaries, so an adult child, a sibling or another relative can generally be the paid caregiver.
A member’s surrogate or authorized representative — the person who makes decisions on the member’s behalf, such as a legal guardian acting in that role — may not also be hired as the paid service provider. Confirm your own situation with your health plan.
Every program also requires the caregiver to pass a background check and, in most states, a short training, and to keep timesheets that match the approved care plan.
How the Hawaii Family Caregiver Pay Is Set and Delivered
Pay is hourly, not a flat daily or monthly stipend. Med-QUEST publishes fee-for-service rates for home and community-based services, and QUEST Integration health plans set the rates paid for self-directed personal assistance, attendant care and respite within those parameters; the service coordinator and the care plan determine how many authorized hours the caregiver may work each week.
Time worked must be recorded through Electronic Visit Verification, which Hawaii requires for both agency and self-direction/consumer-directed personal care in the home and community. The fiscal agent then pays the caregiver as a wage employee.
Pay arrives through a payroll agent as W-2 wages in most consumer-directed programs, or as a stipend through the provider agency in structured models. It does not reduce the parent’s Medicaid benefits.

Yes. VA’s national caregiver programs are available to Hawaii veterans through the VA Pacific Islands Health Care System, including the Program of Comprehensive Assistance for Family Caregivers (PCAFC), which pays a monthly stipend to an approved primary family caregiver, and the Program of General Caregiver Support Services.
