The Idaho Program Behind Idaho Family Caregiver Pay
This is a consumer-directed employment model, not a stipend and not an agency placement. The Medicaid participant becomes the employer of record: a budget is set from the assessed service plan, the participant recruits and hires the relative as a community support worker, sets the schedule, and decides which supports are provided.
A fiscal employer agent, sometimes called financial management services, holds the budget, handles hiring paperwork, withholds and files payroll taxes, and issues the caregiver’s paycheck.

Who Can Be Paid Under Idaho Family Caregiver Pay
Under the A&D Waiver’s self-directed option, the person receiving care may hire a relative as a paid community support worker, including an adult child, sibling, grandchild, niece, nephew, or in-law. A spouse may not be paid. Idaho’s Family Personal Care Services program, which had temporarily allowed spouses and parents of minor children to be paid, was terminated effective July 15, 2025.
A legal guardian’s eligibility to be paid is UNVERIFIED and should be confirmed with the Department before hiring.
Every program also requires the caregiver to pass a background check and, in most states, a short training, and to keep timesheets that match the approved care plan.
How the Idaho Family Caregiver Pay Is Set and Delivered
Pay is hourly wages for documented hours of authorized service, not a flat family stipend. The assessment and service plan set how many hours per week Medicaid will authorize, and that authorization, combined with the individual budget, caps what the caregiver can earn. Under self-direction, the participant and support broker negotiate the worker’s hourly rate within Medicaid’s allowable range for the service.

Pay arrives through a payroll agent as W-2 wages in most consumer-directed programs, or as a stipend through the provider agency in structured models. It does not reduce the parent’s Medicaid benefits.
How to Enroll in Idaho Family Caregiver Pay
First, the parent or spouse applies for Idaho Medicaid for elderly or disabled adults and is found financially eligible. The Veteran Route to Idaho Family Caregiver Pay
Yes. If the aging parent or spouse is an enrolled veteran, the VA’s Program of Comprehensive Assistance for Family Caregivers and the Program of General Caregiver Support Services are available in Idaho through VA Boise health care and other VA facilities serving the state. PCAFC can provide a monthly stipend paid directly to an approved primary family caregiver, plus training, respite, counseling, and in some cases health coverage.

Medicaid waiver payments to a caregiver who lives with the person cared for can be excluded from federal income under IRS Notice 2014-7; payments to a caregiver who lives elsewhere are ordinary wages. Ask the fiscal agent how it reports, and keep the care plan with your tax records.
The Other Route: a Personal Care Agreement
When a parent does not yet qualify for Medicaid but has some savings, the family can put a written personal care agreement in place: the parent pays the caregiver a fair market rate for defined services, with timesheets.
