Rhode Island uses the Supplemental Security Income (SSI) counting rules for these programs, which is the same methodology most states apply. That means gross monthly income is looked at first, then certain amounts are set aside before the state compares your figure to the limit, including a standard disregard applied to earned wages and a general disregard applied to unearned income such as Social Security.
If you are married and living together, your spouse’s income is counted alongside yours and the couple limit is used. Since 2026 Rhode Island applies additional disregards that widen eligibility.
The retroactive rules differ by category, which surprises many families. QMB is not retroactive: it begins after your application is approved, so premiums you already paid before approval are generally not reimbursed, though QMB then covers deductibles and coinsurance going forward.

The QMB Billing Protection Most People Never Use
Federal law forbids any provider who accepts Medicare from billing a QMB enrollee for Medicare deductibles, coinsurance or copays. If a bill arrives, the enrollee does not owe it. Show the QMB card, cite the rule, and if it continues report it to 1-800-MEDICARE or the Rhode Island SHIP. This one protection is worth more than the premium for anyone with regular hospital or specialist care.
Rhode Island Medicare Savings Program and Part D Extra Help
You do not file a separate Extra Help application in Rhode Island. Once the state approves you for any Medicare Savings Program level, Rhode Island reports that enrollment to the Social Security Administration, and you are automatically deemed eligible for the Medicare Part D Low-Income Subsidy, which is what Extra Help is formally called.
The subsidy then attaches to your Part D drug plan and lowers your premium, deductible and copays. If you are not auto-enrolled in a drug plan already, Medicare will assign one, and you may switch plans afterward.

Bring the Medicare card, proof of all income, and — in states with an asset test — bank statements. A decision usually takes a few weeks; the Part B premium refund appears as a larger Social Security deposit once the change reaches Social Security.
Who Typically Qualifies for the Rhode Island Medicare Savings Program
A widow living on one Social Security check. A retired couple whose combined income is modest once one pension stopped. A person under 65 on Medicare through disability. Anyone who receives Supplemental Security Income already qualifies at the top level. Homeowners qualify as easily as renters, because the home is never counted, and in the states without an asset test savings are not counted either.
The Rhode Island Medicare Savings Program Calendar
