Nursing home Medicaid is an entitlement: anyone who qualifies gets it. Home-based care is different. Louisiana delivers it through a waiver or a managed long-term care program with its own name, its own enrollment rules and, in many states, a cap on how many people it serves.

Who Qualifies for Louisiana Medicaid Home Care
Three tests, the same as a nursing home: a medical test, an income test and an asset test. Louisiana uses the nursing-facility level of care as the medical bar, which means the applicant needs hands-on help with daily activities or supervision because of dementia — not merely help with errands.
An applicant must be found to need nursing facility level of care. Louisiana screens first by telephone using the Level of Care Eligibility Tool (LOCET), then confirms with a face-to-face interRAI Home Care (iHC) assessment in the person’s home, which supersedes the phone screen.
The iHC is performed by an assessor who has been trained and certified by OAAS — usually staff of an OAAS-enrolled support coordination agency or an OAAS regional office. The same standard applies to CCW, ADHC, LT-PCS, PACE and nursing home admission.
Financially, the applicant’s own income is measured against the Louisiana medicaid home care limit of $2,982, and countable assets against $2,000. Can a Family Member Be the Paid Caregiver?
Yes. The Community Choices Waiver offers a formal Self-Direction Option, under which the participant (or an authorized representative) becomes the legal employer of the personal assistance worker and recruits, hires, trains, schedules, supervises and may dismiss that worker, including hiring certain relatives such as an adult child. A state-contracted fiscal/employer agent handles payroll, taxes and background paperwork, so the family never cuts the checks itself.
Spouses and legal guardians generally cannot be hired under standard self-direction, though Monitored In-Home Caregiving allows some relatives to serve as the paid live-in caregiver.

What Happens on Assessment Day
The assessment is a visit, usually at the parent’s home, by a nurse or social worker who walks through the day: getting out of bed, bathing, dressing, using the toilet, eating, moving from room to room, taking medication, and whether the person can be left alone safely.
