This is the consumer-directed hourly model, not a stipend and not an agency placement. The waiver participant (or an appointed PDS representative) becomes the employer of record and hires, trains, schedules, and may dismiss the caregiver, while a contracted financial management agency handles payroll, tax withholding, and the actual paycheck.

Who Can Be Paid Under Kentucky Family Caregiver Pay
Under PDS the person receiving care hires their own workers for non-medical, non-residential services, and those workers may be friends, neighbors, or certain relatives such as an adult child, sibling, niece, nephew, or in-law.
A spouse, a guardian, or any other “legally responsible individual” is normally barred from being paid, but PDS allows a narrow exception with prior written approval from the Department, using the PDS Request Form for Immediate Family Member, Guardian, or Legally Responsible Individual as a Paid Service Provider. Approval requires showing unique abilities and outside training or experience, and a weekly hour cap applies.
Every program also requires the caregiver to pass a background check and, in most states, a short training, and to keep timesheets that match the approved care plan.
How the Kentucky Family Caregiver Pay Is Set and Delivered
Kentucky pays by the unit of service, not by daily stipend tier. The Department for Medicaid Services publishes the reimbursement rate for each participant-directed service, such as attendant care and respite, and that rate is what the caregiver’s hours are billed at.
How much the family member can actually earn is bounded by the individual budget written into the plan of care, which is built from the assessed level of need, and by the weekly hour limit that applies when the worker is an immediate family member or legally responsible individual.
Pay arrives through a payroll agent as W-2 wages in most consumer-directed programs, or as a stipend through the provider agency in structured models. It does not reduce the parent’s Medicaid benefits.

The Veteran Route to Kentucky Family Caregiver Pay
Yes, the VA route is available in Kentucky and runs entirely separately from Medicaid. The Program of Comprehensive Assistance for Family Caregivers (PCAFC) pays a monthly stipend directly to an approved family caregiver of an eligible veteran, and the family applies jointly using VA Form 10-10CG through the Caregiver Support Program team at their VA medical center.
