champva and medicare are two separate programs that many military families must carry at the same time. CHAMPVA is the Civilian Health and Medical Program of the Department of Veterans Affairs. It covers the spouse, surviving spouse, or child of a veteran the VA has rated permanently and totally disabled from a service-connected condition. It also covers survivors of a veteran who died from a service-connected condition or in the line of duty. VA explains the eligibility categories on its CHAMPVA benefits page.

Once you become eligible for Medicare, the two programs stop being a choice. VA requires both. This guide describes how champva and medicare fit together, who pays first in each setting, and where to get free help. It is not legal advice.
How Champva And Medicare Works
The core rule is short. If you are eligible for Medicare for any reason, you must have Medicare Part A and Part B to get or keep CHAMPVA. A Medicare Advantage plan (Part C) also satisfies the requirement. VA states this on its CHAMPVA care page.
Age is not the only trigger. People under 65 who qualify for Medicare through disability or end-stage renal disease face the same requirement. So the champva and medicare rule can apply to a surviving spouse at 48 as easily as at 68.
CHAMPVA is almost always the secondary payer. The exceptions are Medicaid, Indian Health Service, State Victims of Crime Compensation, and CHAMPVA supplemental policies. CHAMPVA pays before those four.
You apply with VA Form 10-10d, Application for CHAMPVA Benefits. If you have other coverage, including Medicare, you also file VA Form 10-7959C, CHAMPVA Other Health Insurance Certification. Report changes to your coverage using the same form.
CHAMPVA does not pay your Medicare Part B premium. In 2026 the standard Part B premium is $202.90 a month and the Part B deductible is $283 a year, per the CMS 2026 premium fact sheet. Higher earners pay more.
CHAMPVA has its own costs. There is a $50 per person calendar-year outpatient deductible, capped at $100 per family. The cost share is 25% of the CHAMPVA allowable amount. The catastrophic cap is $3,000 per household per calendar year.
Who Pays First With Champva And Medicare, and Where
The setting decides the order. This table summarizes the common ones. Confirm your own case with the CHAMPVA Help Line at 800-733-8387.
| Setting | Who pays first | What you generally owe |
|---|---|---|
| VA medical facility (CITI program) | Not available to you | If you are eligible for Medicare, you cannot get care through the CHAMPVA In-House Treatment Initiative |
| Civilian doctor, outpatient | Medicare Part B | CHAMPVA may cover the Medicare deductible and coinsurance up to the CHAMPVA allowable amount; a balance can remain |
| Civilian hospital, inpatient | Medicare Part A | CHAMPVA pays second; no CHAMPVA deductible applies to inpatient care |
| Civilian emergency room | Medicare | CHAMPVA pays second on covered charges |
| Medicare Advantage (Part C) | Your Part C plan | CHAMPVA pays second; plan network rules and copays still apply |
| Retail pharmacy with Part D | Medicare Part D | CHAMPVA pays second on covered drugs |
| CHAMPVA Meds by Mail | Not available with Part D | You cannot use Meds by Mail if you have any other drug coverage |
| You also have Medicaid | CHAMPVA | CHAMPVA pays before Medicaid |
One point causes repeated confusion. CHAMPVA is coverage for the family member, not the veteran. The veteran sponsor uses VA health care directly. A Medicare-eligible CHAMPVA beneficiary is treated in civilian settings.
Claims have a deadline. For services on or after January 1, 1994, VA must receive the claim within one year of the date of service. For inpatient care, it is one year from discharge. Use VA Form 10-7959a and attach the Medicare Explanation of Benefits.
What Happens to Champva And Medicare If You Skip Part B
The consequence is direct. If you cancel Part B, your CHAMPVA eligibility ends the same day your Part B coverage ends. VA states this plainly. There is no grace period described on the VA page.
If you are Medicare-eligible and never enrolled in Part B, VA can deny or terminate CHAMPVA. You must be able to show proof of both Part A and Part B.
There is a second cost. Medicare charges a Part B late enrollment penalty of 10% of the standard premium for each full 12-month period you could have had Part B but did not. That penalty lasts as long as you have Part B. See Medicare’s page on avoiding late enrollment penalties.
Getting Part B back is not immediate. The General Enrollment Period runs January 1 through March 31 each year. Coverage begins the month after you enroll. If you already have Part A, you sign up with Form CMS-40B, Application for Enrollment in Medicare Part B.
CHAMPVA is not health coverage based on current employment. That distinction matters, because Special Enrollment Periods generally depend on employer coverage. Confirm your enrollment options with Social Security at 800-772-1213 before you drop anything.
Prescriptions
CHAMPVA offers Meds by Mail for non-urgent maintenance drugs. There is no enrollment, no deductible, and no copay. Medications arrive at your home.
The catch is exclusivity. You cannot use Meds by Mail if you have any other health insurance with prescription coverage. Medicare Part D counts as other coverage. Enrolling in Part D removes your access to Meds by Mail.

If you keep Part D, it pays first at the pharmacy counter. CHAMPVA then pays second on covered drugs, subject to the CHAMPVA deductible and cost share.
VA states that CHAMPVA prescription coverage is at least as good as Medicare Part D. That is called creditable coverage. You can call CHAMPVA and request a creditable coverage letter, which documents the period you were covered.
Keep that letter. Medicare’s Part D late enrollment penalty is calculated on months without creditable drug coverage. Ask CHAMPVA and your SHIP counselor to confirm how your months are counted before you make a change.

Frequently Asked Questions
Does champva and medicare coverage together mean I pay nothing?
No. Medicare pays first, and CHAMPVA may cover what remains up to the CHAMPVA allowable amount. A balance can still be left. The CHAMPVA deductible, the 25% cost share, and your Part B premium all continue to apply.
I turn 65 next year. When do I need Part B?
Your Initial Enrollment Period runs from three months before your 65th birthday month to three months after. Enrolling in that window avoids a gap in champva and medicare coverage and avoids the Part B penalty. Confirm your exact dates with Social Security.
Can I use a Medicare Advantage plan instead of Original Medicare?
Yes. VA states that a Medicare Advantage plan satisfies the Part A and Part B requirement. Your plan’s network and prior authorization rules still govern your care. Report the plan to CHAMPVA on VA Form 10-7959C.
My spouse died. Does my CHAMPVA continue?
Eligibility for survivors depends on the veteran’s VA rating or cause of death. Remarriage and divorce also affect eligibility. Bring your situation to an accredited VSO and to the CHAMPVA Help Line rather than assuming either result.
Key Takeaways: Champva And Medicare
- VA care is not Medicare. champva and medicare matters because VA coverage works only at VA facilities.
- Part B is often required. With champva and medicare, dropping Part B can end other coverage entirely.
- Civilian hospitals need Medicare. Emergency care outside the VA is where champva and medicare protects the veteran.
- Prescriptions can come from both. champva and medicare allows VA pharmacy use alongside Medicare, often at lower cost.
- VA drug coverage is creditable. Under champva and medicare, a veteran can skip Part D without a late penalty.
