Personal Choice is a consumer-directed model: the participant manages an individual service budget, decides what the caregiver is paid within state limits, and the caregiver is paid hourly as a W-2 employee through a fiscal intermediary that handles payroll, taxes and background paperwork.
RIte @ Home is different — it is a shared living arrangement where the person lives with the caregiver, and a contracted RIte @ Home agency recruits, trains, monitors and pays that caregiver. Careforth and Seven Hills Rhode Island currently serve as the contracted agencies.

Who Can Be Paid Under Rhode Island Family Caregiver Pay
Under Personal Choice and the Independent Provider Program, the person receiving care hires, trains and supervises their own worker, and that worker may be a friend, neighbor or relative such as an adult child, sibling, grandchild, niece or nephew. RIte @ Home likewise states that eligible caregivers are relatives and friends.
A spouse is excluded from being the paid caregiver under RIte @ Home, and Rhode Island generally bars a spouse or other legally liable relative from paid caregiving. Whether a legal guardian may be paid is UNVERIFIED.
Every program also requires the caregiver to pass a background check and, in most states, a short training, and to keep timesheets that match the approved care plan.
How the Rhode Island Family Caregiver Pay Is Set and Delivered
The mechanism differs by program. The Veteran Route to Rhode Island Family Caregiver Pay
Yes. If the person needing care is a veteran, the VA’s Program of Comprehensive Assistance for Family Caregivers is available in Rhode Island through the Providence VA Medical Center’s Caregiver Support Program, which pays an approved Primary Family Caregiver a monthly stipend and can include a spouse, adult child or other family member.

Taxes on Rhode Island Family Caregiver Pay
Medicaid waiver payments to a caregiver who lives with the person cared for can be excluded from federal income under IRS Notice 2014-7; payments to a caregiver who lives elsewhere are ordinary wages. Ask the fiscal agent how it reports, and keep the care plan with your tax records.
The Other Route: a Personal Care Agreement
When a parent does not yet qualify for Medicaid but has some savings, the family can put a written personal care agreement in place: the parent pays the caregiver a fair market rate for defined services, with timesheets.
