Three tests, the same as a nursing home: a medical test, an income test and an asset test. Hawaii uses the nursing-facility level of care as the medical bar, which means the applicant needs hands-on help with daily activities or supervision because of dementia — not merely help with errands.
The gateway is a determination that the person meets institutional level of care, commonly described in Hawaii as being “1147 certified” after DHS Form 1147, the level-of-care certification, with Form 1147A used for re-evaluation.
